SINGLE USE PLASTIC POLICY REPORT FORM
On November 16, 2021, the County of Los Angeles Board of Supervisors adopted Policy 3.185 -
Reduce Single-Use Plastics (policy).
The policy states that all County departments must no longer purchase single-use plastic foodware products for use in County facilities and instead use reusable or compostable products.
Products compliant with the policy can be downloaded using the following link:

Departments are required to report foodware purchasing data to Public Works.

Fill in the table starting in the cells with the word “EXAMPLE.” Note that some cells are locked, and you may not be able to edit them. For any questions and to submit the completed file(s), email

For All County Departments (Including Internal Services Department): (DOWNLOAD)
This Excel file is for individual departments to report reusable and single-use foodware purchases. Departments may wish to distribute the file to their divisions/operations to gather data. Departments should consolidate their data into one file per department. Departments are required to submit this file to Public Works monthly.
For Internal Services Department Only: (DOWNLOAD)
This Excel file has two different sheets (tabs).
- The first sheet (tab) is for ISD staff to compile purchasing data for all departments that purchase reusable and single-use foodware (forks, cups, napkins, etc.) through ISD purchasing agreements/service contracts. ISD shall provide this compiled data to Public Works monthly.
- The second sheet (tab) is for ISD staff to fill out for other single-use plastic purchases that are not foodware. ISD shall regularly compile this data and provide it to Public Works upon request.
